The licensing process actually goes through the Greater Fort Lauderdale Convention & Visitors Bureau, aka Visit Lauderdale, which manages film licenses for the county.
They say Jon Bon Jovi himself did not apply for the permit, but that a description of the permit included:
“Video clip at Dania Beach Pier filming a performance; shot of singer playing guitar on edge of pier as sun rises in background. Minimum crew, less than 20 people. “
