An investigation is underway after a woman committed suicide at the emergency department of Perth's Fiona Stanley Hospital (FSH) last week, confirmed WA Health Minister Roger Cook.

Mr. Cook stated that he was not aware of any procedural changes subsequent to the incident.
Executive Director of the Health Consumers Council, Pip Brennan, called for an "effective, transparent and compbadionate response".
"We hope that FSH will guide the family throughout the investigation process," she said.
"We also hope that staff will receive appropriate support as a result of this operation and that it is not an approach of" blame and shame ".
"Instead, we hope that patient safety issues will be solved in a systematic way and that new ways of working will be developed."
