Death certificate of Luke Perry published, buried in Tennessee

Luke PerryHis death certificate confirms what we already know – he died as a result of a major stroke – and he also reveals that his burial place is in Tennessee.

The document was released Wednesday and indicates the cause of death as an "ischemic stroke" – a stroke – occurred 5 days before his death on Monday, March 4 at 12:44.

According to the certificate obtained by TMZ, Luke's fiancée, Wendy Bauer, is listed as his closest relative, or "informant". Luke's body was also transported to Dixon, Tennessee, where he was buried on Monday, March 11th.

Luke was a part-time resident of Tennessee and had a farm there since 1995.

TMZ broke the story – Luke dead in Burbank at St. Joseph's Hospital, where he had been since.

As we reported for the first time, paramedics rushed to Luke's at Sherman Oaks on February 27 suffered the blow. He was responsive and spoke when paramedics arrived, but his condition deteriorated. The doctors calmed Luke in the hope that his body could recover, but the damage was too great and his family made the decision to remove him from the survival system.

Luke was 52 years old.
