It appears that Sam's bladder fills over time, as shown by the head-up display indicating the number of milliliters of urine in his bladder. According to Kojima, urination can be a tool against enemies. Although, the details of the way remain obscure.

After Sam had finished peeing, a weird mushroom sprouted from the ground. Kojima noted that the world of Death Stranding was connected, so if more players urinated on the field, this mushroom would develop fully and become a useful consumable that you can recover.
