Experts at the press conference on the twelfth diabetes conference of the German Diabetes Society (DDG) and the 34th Annual Congress of the German Society of Obesity (DAG) will discuss measures most effective for patients Tuesday, November 6, 2018, in Berlin.
"People with type 2 diabetes suffer from insulin resistance, which means that if their pancreas produces insulin, it is released in too small amounts or does not work properly with the target cells." says Dr. Dr. med. med. "In addition to a genetic predisposition, obesity, particularly obesity of organs and muscles, promotes insulin resistance," said Pfeiffer.
With a healthy and calorie restricted diet and proper exercise, patients with type 2 diabetes can often improve insulin resistance and treat their diabetes in the long run without medication. "A diet and low calorie exercises stimulate the burning of fats in the organs, such as the liver and muscles, and increase their sensitivity to insulin," says Professor of the Department of Endocrinology, Diabetes and of Nutritional Medicine of Charity – Universitätsmedizin Berlin. Therefore, in the treatment of type 2 diabetes, nutritional therapy as well as the increase in physical activity are prioritized. Drug treatment should be initiated according to guideline (2) only if these measures fail.
The DiRECT study published in the journal "The Lancet" confirms the effectiveness of such a lifestyle-oriented intervention: 298 obese patients diagnosed with type 2 diabetes there are six years maximum. The study participants participated in an intensive weight reduction program (mainly through a high-calorie diet with 500-800 kcal per day) in order to lose 15 kg or more. Half of the participants in the study were able to normalize their sugar metabolism, that is to say to obtain a remission of diabetes. The more effective the treatment, the more successful the treatment was: in patients who could reduce their weight by more than 15 kilograms, a remission of diabetes was about 85%. In patients with weight loss of about ten kilograms, the likelihood of an improvement in symptoms has dropped to around 50%. In patients who declined only about five kilograms, the success rate dropped to about 20 percent (3).
"The prospect of controlling type 2 diabetes without drugs or insulin syringes is an important motivation for patients, but many patients struggle to change their lifestyle in the long term," said Pfeiffer. "It's all the more important to offer these patients an early and early five-year post-diagnosis program for an intensive weight loss program." Improving beta cell function and achieving good insulin sensitivity both fasting and after meals, is a weight reduction of about 15 kilograms the most promising. "Our experience shows that such a low-calorie diet, which can easily be achieved with a formula to ensure the intake of all essential nutrients, can be achieved in more than eight to twelve weeks by more than 90% of concerned, "said the expert. "In order to maintain weight, an intensive follow-up program with a nutritional change for a healthy diet is necessary."
However, a remission is not synonymous with healing. "We are talking about a remission, when the symptoms of a disease are brought back to normalization – this may be the case both temporarily and permanently," says Prof. Dr. med. med. Jens Aberle, president of the German Diabetes Association. "It's possible that type 2 diabetes will reappear after a while," said the medical director of the Hamburg-Eppendorf University Hospital.
