We know that the future Steve is back in time to be with Peggy, as shown in the last scene of End of Game where they finally had the dance that they owed. In this scene, Peggy and Steve are finally together and dance in what seems to be their home.

In Winter SoldierSteve visits the exhibition dedicated to him at the Smithsonian Institution. He watches a video recording of Peggy dating back to 1953, where she explains how, during the war, Captain America rescued a group of men, including a man who would become her husband. At first it looked like Cape Town had rescued a soldier at random, but could she have talked about Captain America himself since, technically, he's also running away?

In the same movie, Steve visits 93-year-old Peggy in the retirement home where she learns she has Alzheimer's disease. We see pictures of Peggy with her children on the table, but no photos of her husband. She tells Steve that she has lived a long and rewarding life before going through an episode where she thinks Steve has come back to her. Could it be because she's married to the love of her life, Steve Rogers? Plus, as she grew older with Steve, seeing a young Steve Rogers would make her think he would come back to her.

But that does not make us wonder why she would still have a photo of Steve on her desk if she was already married to someone else with a family to her.

The best bet is Steve who came back to the chronology between 1947 (the end of the Agent Carter series) and 1953 (where she revealed that she was married, according to the Winter Soldier Smithsonian) with Peggy. At the end of Avengers: End of Game, a 1940s-style car went through the scene, confirming the era. This period would give them time to reconnect and get married.

Many can question Steve's slow physical aging. By the time he appeared in 2023 to End of Game, Steve would have been over 100 years old. But it is likely that the Super Soldier serum that granted her powers also slowed her aging process.

Another question: where was the future Steve during Peggy's time in the retirement home and at his funeral? The future Steve would have known when the current calendar would visit him because he had already lived. He would know that he will have to keep his distance to keep the chronology intact. That's also why the photos on Peggy's nightstand are about her and her kids.

But what about Peggy's granddaughter, Sharon Carter (Emily VanCamp), who had a romantic relationship with Steve? Would not she recognize "Uncle Steve" as Captain America? Perhaps. But it is more likely that she only thinks that they are alike but do not suspect that they were actually the same person, especially since "Uncle Steve" was a little more older than the Captain America that she knows.

The same could apply to the entire social and business circle of Steve and Peggy. While everyone thinks that Captain America is definitely gone, no one suspects that the future Steve is actually him. Peggy and the future Steve would have taken great care to keep his true identity secret.
