The location of the fight has not been determined yet, but many reports have announced that it will take place at the T-Mobile Arena in Las Vegas.
For Alvarez, World Boxing Council and World Boxing Association champion (51-1-2, 35 KO), this fight will be the first since his knockout victory over Britain's Rocky Fielding last month.
"I am happy to announce my next fight at the Cinco de Mayo weekend party," said Alvarez, 28, in a statement.
"I am going to unify my middleweight titles against Daniel Jacobs on one of the two most important dates that I have, I have no doubt that I will be victorious and that I will be on the verge of becoming the undisputed middleweight world champion. "
American Jacobs, nicknamed "The Miracle Man" after winning his battle against bone cancer, defeated Sergiy Derevyanchenko by split decision when he last defeated in October to win the title of the International Boxing Federation.
Jacobs was diagnosed with some form of bone cancer in May 2011 and was left partially paralyzed. After radiotherapy treatments, an operation to remove a tumor wrapped around his spine and an arduous rehab, he returned to boxing at the end of 2012.
A 31-year-old native of Brooklyn (35-2, 29 KO's), he is already considered one of the best middleweights of his time and his clash with Alvarez will be the most important of his career.
"This is the opportunity I was expecting – the opportunity to do great things in the ring," said Jacobs.
"I've always thought I could beat Canelo, and on May 4th … I'll have the opportunity to play." (Report by Frank Pingue in Toronto edited by Toby Davis)
