New Yorkers will soon get their COVID-19 vaccination cards back after the city announces that proof of vaccination will be required for some indoor activities – but what if they misplaced the important file of their status?

The paper card contains vital information, including the brand of vaccine received as well as the dates a person was immunized – and this will be the key to getting into the Big Apple’s indoor restaurants, entertainment venues and gyms from August 16.

While residents of Gotham cannot get another vaccination card at this time, there are other ways to prove that they have been fully vaccinated against the virus.

Here’s what New Yorkers can do if they’ve lost the card:

Get an official record of your vaccination history online

Residents can visit the My Immunization Record page on the city’s health department website to obtain an official copy of their immunization record.

To access their records online, they can enter their cell phone number, email address or IDNYC number.

Printouts of the report can be used for school enrollment and verification, college admissions, camp enrollment and as a COVID-19 vaccination record, according to health officials.

Request an official record of your vaccination history by mail

New Yorkers can request that an official record of their immunizations be mailed to them by printing and completing a request on the city’s health department website.

The application must be mailed with a copy of a valid photo ID, such as a driver’s license or passport.

It can be posted at the Citywide Immunization Registry, 42-09 28th Street, 5th Fl., CN 21, Long Island City, New York.
