Re-registration Procedure at the University of Sumatera Utara

2. Delivery of UKT files by mail or delivery to the USU Academic Bureau on July 4 and 12, 2018, enclosing:

a. Last month's pay slip or income statement of the party who financed 1 sheet

b. Graduated information from the beginning of the school of the potential pupil

c. Copy of parent / guardian identity card 1 sheet

d. Photocopy of parent KK / guardian sheet 1

e. Copy of parent / guardian sheet of NPWP 1

F. Copy of proof of payment Guardian / parent from last year of the UN 1 sheet

g. Photocopy of motor vehicle tax proof last year parent / guardian 1 sheet

h. Photocopy of the electricity bill last month where the parent / guardian residence sheet 1

i. Photocopy of last month's water account evidence on parent / guardian's residence sheet 1

j. Photocopy of the last year of residence of the parent / guardian 1 personal income tax return

k. Print photos of the residence house front view a leaf. House number searched, RT / RW and other identities.

4. Drug-free examination at the USU Education Hospital from 9 to 13 July 2018

5. The payment of UKT / SPP was made on July 18 and 23, 2018 in the banks BNI, Mandiri, Bank Sumut and BTN.
