Al-Hazem, whose Syrian wing, Youssef Qulfa, joined the Al-Qadisiyah team during the ongoing transfer.
Aged 25, he joined Al Hazm last summer in the UAE, where he played 17 games for the first team but failed to score.
Al-Hazem said in an official statement: "The club leadership has accepted the request of its counterpart in Qadisiya to transfer Syrian international player Youssef Qulfa."
