Phoenix police told KTVK, a subsidiary of CNN, that the fire truck was heading for a fire near a structure when it collided with a pickup truck around 9:15 am local time.

Sgt. Vince Lewis told reporters that the deceased was an adult man, an adult woman and a child aged about 6 months who sat in the child seat at the back of the vehicle.

The cause of the accident was the subject of an investigation Sunday afternoon, said Lewis. Investigators from the driving homicide unit did not determine the cause.

"It seems, as a preliminary, at that time, that the van turned left in front of the fire truck," he said. "But again, I want to give our investigators time to solve this problem."

The truck had its headlights and sirens on, KTVK reported.

Lewis said three firefighters were taken to the hospital, but their condition was stable Sunday afternoon.

The images of the scene showed the fire truck on the side and the other damaged vehicle in front.

The accident occurred near 29th Avenue and Bethany Home Road.
