Rey Mysterio recently returned to WWE and did not have many prestigious opportunities to show what he can do. He lost in the World Cup tournament and failed to make the full course for the Blue Team at Survivor Series, and a post-match assault of Randy Orton the week last to SmackDown left him defeated and without a mask.

So, when Mysterio showed up at the Starrcade event in Cincinnati, fans were a little shocked to see that Mr 6-1-9 was wearing a neck protector. This could be a sign of a legitimate injury, but it would still play until Randy Orton's attack, and hope that Mysterio just plays the attack on The Viper's script.
During his segment at Starrcade, Mysterio and Shinsuke Nakamura participated in the Miz TV segment, then Nakamura defeated Mysterio, forcing a referee to start and start the game with total control of the player.
Nakamura eventually lost via DQ after the Miz intervened, then Rusev rushed for the save and prepared for the team match. After all, the babyface team of Mysterio and Rusev won.
Chances are that since Mysterio competed in Starrcade, he's fine. After all, WWE will not let their talents work at all, let alone if they need a neck brace. But Mysterio wearing a neck brace shows that they are trying to preserve the continuity of the weekly TV show.
