Hy-Vee confirmed that staff at one of its Minnesota stores improperly diluted a Pfizer COVID-19 vaccine which was then administered to 62 patients.
The grocery chain, whose pharmacies are among three in Minnesota licensed to administer vaccines under a federal program, said the incident occurred recently at its Riverfront Hy-Vee pharmacy in Mankato.
He said there was an oversight when the first dose of the vaccine was given to 62 patients.
As he explains, “Two pharmacy staff used vaccines mixed with sterile water injection diluent – which is commonly used to dilute injections, rather than the saline diluent supplied by Pfizer.
The channel says it has consulted with third-party doctors working on the vaccination process and has “received confirmation that there is no reason for medical concern.”
“No patient has also reported side effects,” he added.
“As soon as we were informed of the incident, we began to personally call the affected patients,” he continued. “We have contacted the manufacturer and are working with each of the patients on the next steps. We have also reported this information to the appropriate regulatory authorities and have re-trained all of our pharmacy staff there.
“We remain committed to playing an important role in delivering the COVID-19 vaccine to our local communities.”
