Nearly 2,000 doses of the Moderna COVID-19 vaccine have been spoiled at the Jamaica Plain Veterans’ Medical Center.

VA Boston Pharmacy staff found Tuesday that the freezer containing the doses had failed, leaving 1,900 compromised shots, according to a statement from a spokesperson for the VA Boston Healthcare System.

The freezer was “in a safe place and had an alarm system installed,” the statement said.

“The plug was found loose after a contractor accidentally removed it during the cleanup,” officials said. “An investigation is underway to determine the cause of the incident and why the monitoring and alarm system did not function as expected.”

Despite the loss, the spokesperson said reconstitution doses were being processed and the health system did not anticipate any disruption to its vaccine rollout plan.
