When a person is sentenced to a mandatory hospital order, they are forced to seek treatment in a closed clinic. The SCT initially lasts two years. After that, a judge can extend the TBS every one or two years.

The Tbs may sometimes be on leave. This allows practitioners to see if treatment is progressing well and if a person can return to society. The leave has four phases:
